Our support in the Community Service is a free service which endevours to assist people to achieve thier own health and lifestyle preferences.
A support Worker helps to identify ways in which each person can achieve their goals.
The application process involves a referral from a health practitioner and an eligibility assessment.
The Community Support Workers aim to provide oppourtunities for people to develop:
- Skills to access recreation and leisure activities
-Life skills
-Employment and vocational oppourtunities
-Support to get out and about and broaden horizons
-Ways to identify and use available resources
Skills to access housing in the community
Techniques for staying well.
Open Monday to Friday 8:30am-4:30pm, hours outside of these by negotiation.
